You will use several forms, depending on what you are trying to do and your relationship with the abuser. The personal protection order (PPO) forms are provided only to show you what each looks like and what information you must include. Your court might not accept a print-out of the SCAO forms from this website. If so, when you file for your PPO you may have to use the original forms the court clerk gives you.

If your court lets you use a printed-out form, many of the PDF documents listed have fill-in-the-blank fields. Just type in the required information and check necessary boxes, and print it all out! (Use the Tab key to go from field-to-field.)

If you need a PPO for a minor, the minor cannot file on his/her own behalf. The Court must appoint a “next friend” (who can be one of the minor’s parents) to represent the minor.

This is Shiawassee County’s version of the packet that you would use if you and the person you want restrained have a ‘domestic relationship’ (spouse, former spouse, current or former household resident, child in common, dating relationship, etc.)

This is Shiawassee County’s version of the packet that you would use if you want to restrain another person from stalking or harassing you and you do not have a ‘domestic relationship’ (as defined above)

If you and the person you want restrained have a ‘domestic relationship’ (spouse or former spouse; current or former household resident; child in common; dating relationship).

Companion form to CC 375 (PDF) , becomes the actual Order signed by the Judge. You must fill out the informational boxes at the top (items “A” through “D”).

If you and the minor you want restrained have a ‘domestic relationship’ (current or former household resident; child in common; dating relationship).

If you want an order to restrain another person from stalking or harassing you and you do not have a domestic relationship with this person.

If you want an order to restrain a minor from stalking or harassing you and you do not have a domestic relationship to this person.

Companion form to CC 377M (PDF) , becomes the actual Order signed by the Judge. You must fill out the informational boxes at the top (items “A” through “E”).

Use this form if you want an order to restrain a person over the age of 18 from sexually assaulting you or threatening to sexually assault you, you do not have a domestic relationship with this person, and that person has: previously been convicted of a sexual assault against you, or subjected you to, threatened you with, or placed you in reasonable apprehension of sexual assault, or previously been convicted of furnishing you, a minor, with obscene material, or if you are a minor and this person has furnished you with obscene material.

If you want an order to restrain a person under the age of 18 from sexually assaulting you or threatening to sexually assault you, you do not have a domestic relationship with this person, and that person:

  • Is subject to an order of disposition or other adjudication based on a sexual assault against you
  • Has subjected you to, threatened you with, or placed you in reasonable apprehension of sexual assault
  • Is subject to an order of disposition or other adjudication based on furnishing you, a minor, with obscene material
  • Has furnished you, a minor, with obscene material

If you did not request an ex-parte order, or the judge refuses to issue an order without a hearing.

Order signed by the judge if your petition for PPO, or proofs in a contested hearing, do not merit issuance of a PPO.

If you do not want to continue with your Motion for a PPO and the order has not yet been signed by the Judge. (This form is rarely used.)

If you want to change the terms of, extend the duration of, or dismiss a PPO that has been signed by a Judge. The Respondent can use this form to try to change the terms of, or dismiss, the PPO.

Companion form to CC 379 (PDF) and A 379SP (PDF) , filled out by the Judge when the Petitioner or Respondent changes the terms of, or dismisses, the PPO.

Use if the Respondent (i) has been served with the PPO, (ii) has violated the PPO, (iii) has not been arrested by the police for the violation, and (iv) you want the court to order him in to court to answer to a contempt charge.

Companion form to CC 382 (PDF) ,filled out by the Judge after a show cause hearing claiming that the respondent violated the PPO.

Use this form after the defendant has been served with a copy of the PPO.
This form must be filed with the court after the other party is served!
